Welcome to the Montgomery County Clerk's online ordering system for certified copies of birth certificates, death certificates, and c
opies of a marriage license.

Online requests will be processed during normal business hours.

This is an official request for a Vital Record from the Montgomery County Clerk's Office. Vital records include birth, death, and marriages for Montgomery County. The information contained in this request should be considered private. Please complete all information in full and then finalize the order process through payment of the 'order fee'.

 

Processing Information:

  • You will receive an email notification when your application has successfully been submitted into the online ordering system.
  • You will receive a second email notification and a phone call when our office has notified you of needed documents or questions regarding your order.
  • Once your application has been Approved, a confirmation email will be sent to notify you that your document is ready for pick-up, or will be mailed the following business day.
  • If your application has been Denied, you will be notified by email.  

The fee payment will be paid through Permitium Payments.  The charge will show on your credit card statement as 'PermVitalRecs'. This is a secure and trusted provider of payment services.

 

Office Pickup

Please note that if you opt to pick up your records upon the approval of your application, you MUST pick them up at our Conroe location.  The contact information and hours of operation are listed below:

Note: Orders not picked up within 10 business days of the order being completed will be mailed to the address on the application once the address is verified. Valid ID will be required for pickup; only applicant or person listed on the record is allowed to pick-up.

Pickup is NOT automatic. Please wait for approval email to pick-up your order. Please bring Identification.

 

 

Birth Records

Birth records for Montgomery County Clerk's Office and surrounding areas are available back to 1906.

Birth certificates are only available through this site to those born in the state of Texas.

The Montgomery County Clerk’s Office maintains long form birth certificates for individuals born within the Montgomery County registration district.

This registration district does not include parts of the City of Kingwood. Long form birth certificates for births occurring in areas of Kingwood (HCA Healthcare-Kingwood) must be obtained from the City of Houston. Our office can provide the abstract version.

 

  • Long form Birth Certificate - Most comprehensive birth record. It is a copy of the original birth certificate. It will also show a history of corrections that have been made to the birth record. This form is often used for requesting passports.

 

  • Short form Birth Certificate - An abstract of the birth record for a Texas birth. This birth certificate will only show current information for the child's name, date of birth, place of birth, sex, and name of parent(s). This form will not show a history of corrections. This form is often used for school records and is acceptable for most purposes.

 

Newborn Birth Certificates:

It may take 1-3 weeks for the birth certificate to be generated by the birthing facility and DSHS-Austin. Once received by our office your application will be processed and you will be notified of delivery or order pick up.

 

Death Records:

Certificates for all deaths recorded in Montgomery County in 1903 to the present are available.

Death Records are available only for decedents who pass within Montgomery County. This registration district does not include parts of the City of Kingwood. Death certificates for deaths occurring in areas of Kingwood (HCA Healthcare-Kingwood) must be obtained from the City of Houston.

 

Marriage Licenses:

Marriage records in Montgomery County exist from 1838 to present.

Montgomery County can only issue certified copies of your marriage license if you purchased your license with Montgomery County. This is not where you got married.  If you purchased your marriage license in another county, please contact that county to purchase certified copies.

Warning: The penalty for knowingly making a false statement in this form can be 2-10 years in prison and a fine up to $10,000 (Health and Safety Code 195.003)
